More ideas for reducing calories?

The latest issue of US News and World Report has a neat article (in which I am quoted) with some interesting ideas about how restaurants can help customers cut calories. Would any of them work?

I am a big supporter of nutrition listings on restaurant menus. My husband has high cholesterol, and menus do not provide the information he needs to make good choices. Waitstaff, while wanting to be helpful, are not trained for this.
I tried healthydiningfinder.com and found it only minimally helpful. Most, if not all, listings are for chain restaurants, and quite often you need to remember to ask that the salad dressing be changed or (in the case of Kona Grill) that the rice be switched out for steamed vegetables or make some other modification in the menu.
